It is believed to be an Anglicized form of the Irish Gaelic name 'Ó Cusha', which translates to 'descendant of Cusha'. The name Cusha itself is thought to be derived from the Old Irish word 'cuas', meaning 'curved' or 'bent'.

One of the earliest known references to the CUSHWA name can be found in the Petty's Census of Ireland from 1659, which recorded several families with the surname living in County Fermanagh.
