The surname Nagao is a Japanese name that originated in the late 15th century to early 16th century. It is believed to derive from the Japanese word "naga" meaning long and "o" meaning tail or stream, possibly referring to a long stream or river. The name is associated with the Ishikawa Prefecture region of central Honshu Island.

One of the earliest recorded references to the Nagao name dates back to the Sengoku period (1467-1615). A powerful samurai clan known as the Nagao clan controlled lands in the Echigo Province (modern-day Niigata Prefecture). The head of this clan, Nagao Kageharu, lived from 1429 to 1487 and was a skilled military strategist.

In the late 16th century, a notable figure named Nagao Tamekage served as a retainer to the powerful warlord Tokugawa Ieyasu. Nagao Tamekage played a vital role in the Battle of Sekigahara in 1600, which solidified Tokugawa's control over Japan and marked the beginning of the Edo period.
