The surname PICANSO originated in the northern region of Spain around the 12th century. It is believed to have derived from the Latin word "piccatus", meaning "adorned with small dots or speckles". In the early days, the name was often spelled as "Picansio" or "Picancio" in various Spanish records and documents. One of the earliest known references to the name can be found in a 13th-century manuscript from the Monastery of Santo Domingo de Silos, where a certain "Juan Picansio" is mentioned as a landowner in the village of Quintanar de la Sierra.

By the 15th century, the name had spread to other parts of Spain, including the regions of Andalusia and Catalonia. In a 1492 census record from the city of Seville, there is an entry for a family headed by "Pedro Picanso", a merchant and trader.
