It is believed to have derived from the Italian word "pisano," which means someone from the city of Pisa. The name likely emerged in the 12th or 13th century, during the height of the maritime republic of Pisa, when many Pisans were establishing settlements and trading outposts throughout the Mediterranean region.

One of the earliest recorded instances of the Pisacano surname can be found in the medieval Codex Diplomaticus Cavensis, a collection of documents from the Abbey of Cava de' Tirreni in Campania. The codex contains a reference to a certain "Petrus Pisacano" in a legal document dated around 1230.

In the 14th century, the name appears in the historical records of the Kingdom of Naples, where a nobleman named Goffredo Pisacano is mentioned as a courtier in the service of King Robert of Anjou, who reigned from 1309 to 1343.
